The Ibis program, developed by Senscio Systems, is a self-management support program designed for those with the most complex care who need frequent support to maintain their health.
Who qualifies for the Ibis Program?
Individuals meeting the following criteria can choose to join the Ibis Program:
- Have Medicare Part A and Part B or C as primary insurance.
- Have two or more chronic conditions expected to last 12 months or more and that place the patient at significant risk of death, acute exacerbation, decompensation, or other functional decline.

How does the program work?
When individuals join the Ibis program, they agree to receive self-management support from a Chronic Care Clinician. They receive an interactive device in their home that will:
With Ibis, individuals with complex chronic health see fewer hospital admissions and are able to age in community more easily.
Who pays for the program?
The Chronic Care Clinician bills Medicare for the services provided by the Ibis Program. Beneficiaries are responsible for Medicare co-payments.
